• High Spinal (Respiratory Arrest): If the patient lies flat too quickly or the table is tilted head-down excessively, the drug can travel too high and paralyze the diaphragm.
• Wrong Route (FATAL): NEVER inject Intravenously (IV). Bupivacaine is cardiotoxic and can cause immediate cardiac arrest.
