Dosage & Administration
Recommended Dosage (Strictly as per Physician):
- Route: Deep Intramuscular (IM) Injection ONLY. (Must be injected deeply into the upper outer quadrant of the gluteus maximus).
- CRITICAL WARNING: NEVER ADMINISTER INTRAVENOUSLY (IV). Intravenous administration can cause severe vascular damage and systemic shock.
- Standard Adult Dose: 20 mg once daily.
- Acute Gout / Severe Trauma: 40 mg once daily as a single dose, or in divided doses (20 mg twice daily) for the first 1 to 2 days, strictly transitioning to oral therapy thereafter.
- Duration constraint: IM therapy should generally not exceed 2 to 3 days. Patients should be switched to oral Piroxicam as soon as clinically feasible.
Safety Warnings (CRITICAL):
- BLACK BOX WARNING (Gastrointestinal Risk): NSAIDs cause an increased risk of serious gastrointestinal adverse events, including bleeding, ulceration, and fatal perforation of the stomach or intestines. Risk is significantly higher in the elderly.
- BLACK BOX WARNING (Cardiovascular Risk): NSAIDs may cause an increased risk of serious cardiovascular thrombotic events, myocardial infarction (heart attacks), and stroke, which can be fatal. Contraindicated for the treatment of peri-operative pain in the setting of coronary artery bypass graft (CABG) surgery.
- Severe Skin Reactions: Piroxicam carries a higher risk than many other NSAIDs for severe skin reactions, including Stevens-Johnson Syndrome (SJS) and Toxic Epidermal Necrolysis (TEN). Discontinue immediately at the first sign of skin rash.
