Dosage & Administration

Recommended Dosage (Strictly as per Oncologist):

• Standard Dose: 500 mg/m² administered on Day 1 of each 21-day cycle.
• MANDATORY PRE-MEDICATION (Vitamin Supplementation): To reduce severe hematologic and GI toxicity, patients MUST receive:
  • Folic Acid: 400 to 1000 mcg orally daily, beginning 7 days prior to treatment.
  • Vitamin B12: 1000 mcg IM injection 1 week prior to the first dose and every 3 cycles thereafter.
  • Corticosteroids: Dexamethasone 4 mg twice daily on the day before, day of, and day after infusion to prevent severe skin rashes.

Safety Warnings (CRITICAL):

• Myelosuppression: Dose-limiting toxicity resulting in neutropenia, thrombocytopenia, and anemia. Regular CBC monitoring is mandatory prior to each cycle.
• Renal Clearance & NSAIDs: Pemetrexed is primarily excreted by the kidneys. Avoid NSAIDs (like Ibuprofen or Diclofenac) 2 to 5 days prior to, during, and 2 days after administration, as they delay drug clearance and increase fatal toxicity.
• Renal Impairment: Contraindicated in patients with severe renal impairment (CrCl < 45 mL/min).
