This formulation contains Furosemide (Frusemide), a sulfonamide-derived “loop” diuretic that is the gold standard for rapid fluid removal.

  • Mechanism (The “Torrent” Effect): Furosemide acts on the Thick Ascending Limb of the Loop of Henle in the kidney. It inhibits the Na+/K+/2Cl- co-transporter, preventing the reabsorption of sodium and chloride. This causes a massive excretion of water (“torrential diuresis”) along with sodium, chloride, potassium, and magnesium.
  • Acute Pulmonary Edema: In heart failure, fluid fills the lungs (drowning the patient). IV Furosemide acts within 5 minutes to vasodilate veins (reducing preload) and then force kidneys to excrete fluid, rapidly clearing the lungs and restoring breathing.
  • Renal Failure: It is effective even when Glomerular Filtration Rate (GFR) is low, making it the diuretic of choice for Chronic Kidney Disease (CKD).

Dosage & Administration

Recommended Dosage (Strictly as per Physician):

  • Route: Intravenous (IV) or Intramuscular (IM) Injection.
  • Adults (Standard): 20 mg to 40 mg IV/IM initially. Repeat after 2 hours if needed.
  • Pulmonary Edema: 40 mg Slow IV injection over 1-2 minutes.
  • Renal Failure: High doses (250 mg to 1000 mg) infused slowly (Max 4 mg/min).
  • Administration Rate: DO NOT EXCEED 4 mg/minute. Rapid injection causes permanent deafness.

Safety Warnings (CRITICAL):

  • Ototoxicity (Deafness): Rapid IV injection can cause tinnitus or permanent hearing loss, especially in renal failure or when combined with Aminoglycosides. Inject Slowly.
  • Hypokalemia (Low Potassium): Causes severe loss of Potassium. Can trigger fatal arrhythmias. Monitor electrolytes and supplement Potassium.
  • Sulfa Allergy: Related to sulfonamides. Use with caution in patients with severe sulfa allergy.
  • Dehydration: Excessive diuresis can cause circulatory collapse and thrombosis.
