• Mechanism (DNA Cross-Linking): Oxaliplatin undergoes non-enzymatic conversion in physiologic solutions to active derivatives that bind to DNA. It forms both inter- and intra-strand platinum-DNA crosslinks. The unique bulky DACH ring prevents mismatch repair proteins from binding to the damaged DNA, making it highly effective even in tumors that are resistant to Cisplatin and Carboplatin.
• The FOLFOX Backbone: It is the global standard of care when combined with 5-Fluorouracil (5-FU) and Leucovorin (the FOLFOX regimen) for the adjuvant treatment of Stage III colon cancer and the treatment of advanced/metastatic colorectal cancer.
• Synergistic Action: Oxaliplatin down-regulates thymidylate synthase expression, acting synergistically with 5-FU to trigger apoptosis in gastrointestinal tumor cells.

Therapeutic Indications (Human Use)

Dosage & Administration

Recommended Dosage (Strictly as per Oncologist):

• Route: Intravenous (IV) Infusion ONLY. Administered over 2 hours.
• Standard Dose (FOLFOX): 85 mg/m² IV given on Day 1, every 2 weeks.
• FATAL DILUTION ERROR WARNING: NEVER DILUTE WITH NORMAL SALINE (0.9% NaCl). Chloride ions rapidly degrade Oxaliplatin. It must ONLY be diluted in 250 ml to 500 ml of 5% Dextrose Injection (D5W).
• Flushing: Flush the IV line with D5W prior to and after administration. Do not mix with alkaline medications (like 5-FU) in the same IV line simultaneously.

Safety Warnings (CRITICAL):

• Cold-Induced Neuropathy: A hallmark toxicity of Oxaliplatin. Causes acute, reversible sensory neuropathy exacerbated by exposure to cold (e.g., drinking cold liquids, touching cold items can cause severe pain or spasms in the throat/hands).
• Anaphylactic Reactions: Hypersensitivity reactions can occur within minutes of administration. Epinephrine, corticosteroids, and antihistamines must be on hand.
• Myelosuppression: Neutropenia, thrombocytopenia, and anemia. Routine complete blood counts (CBC) are mandatory.
• Pulmonary Fibrosis: Rare but fatal interstitial lung disease has been reported. Discontinue if unexplained respiratory symptoms occur.
