Dosage & Administration
Recommended Dosage (Strictly as per Physician/Program Guidelines):
- Route: Oral.
- Lymphatic Filariasis: 6 mg/kg body weight daily in divided doses for 12 days.
- Tropical Pulmonary Eosinophilia: 6 mg/kg body weight daily in divided doses for 21 days.
- Loiasis: Requires careful titration to avoid severe reactions.
- Day 1: 50 mg.
- Day 2: 50 mg three times daily.
- Day 3: 100 mg three times daily.
- Day 4–21: 3 mg/kg three times daily (approx 9 mg/kg/day).
- Administration: Best taken after meals to reduce gastric irritation.
Safety Warnings:
- Mazzotti Reaction (Systemic): The rapid death of microfilariae can cause a severe allergic-like reaction (fever, headache, muscle pain, rash, swollen lymph nodes). This is a sign the drug is working but can be distressing. Antihistamines or corticosteroids may be needed.
- Onchocerciasis (River Blindness) – CONTRAINDICATED: DEC should NOT be used for Onchocerca volvulus infections as it can cause a severe reaction leading to permanent blindness. Ivermectin is the drug of choice there.
- Renal Impairment: Dosage reduction required as the drug is excreted by the kidneys.
