This formulation contains a strategic 1:1 combination of a broad-spectrum penicillin (Ampicillin) and a penicillinase-resistant penicillin (Cloxacillin).

• Mechanism (Cell Wall Collapse): Both antibiotics act by binding to Penicillin-Binding Proteins (PBPs) inside the bacterial cell wall, inhibiting the final stage of peptidoglycan synthesis (transpeptidation). This weakens the cell wall, causing the bacteria to burst (lysis) due to osmotic pressure.
• The Synergy (Why Combine?):
  • Ampicillin: Excellent coverage against Gram-negative bacteria (E. coli, H. influenzae) and some Gram-positives, but it is destroyed by beta-lactamase enzymes produced by Staphylococcus aureus.
  • Cloxacillin: While less active against Gram-negatives, it is highly resistant to staphylococcal beta-lactamases. By combining them, Cloxacillin “protects” the regimen against resistant Staph, while Ampicillin handles the broader spectrum.

Safety Warnings (CRITICAL):

• Penicillin Allergy (ANAPHYLAXIS): CONTRAINDICATED in patients with a history of penicillin hypersensitivity. A standard sensitivity test (AST) must be performed before the first dose.
• Cross-Sensitivity: Caution in patients allergic to Cephalosporins.
