• Mechanism: Albendazole works by binding to the beta-tubulin protein in the parasite’s cells. This inhibits tubulin polymerization, preventing the formation of microtubules. Without microtubules, the worm cannot absorb glucose or transport nutrients. This energy depletion effectively “starves” the parasite to death.
• Dual Utility:
  • Intestinal Worms: For worms living in the gut (Roundworm, Hookworm), systemic absorption is not required; the drug acts locally.
  • Systemic Infections: For larvae encysted in tissues (Neurocysticercosis, Hydatid Cyst), the drug is absorbed (enhanced by fatty food) to penetrate the brain or liver cysts.
• Ovicide: It kills not only the adult worms and larvae but also the eggs, breaking the cycle of transmission.

Dosage & Administration

Recommended Dosage (Strictly as per Physician/WHO):

• Route: Oral.
• Intestinal Worms (Adults & Kids >2 yrs): 400 mg as a single dose.
• Pinworm: 400 mg single dose, repeat in 2 weeks.
• Systemic Infections (Hydatid/Neurocysticercosis): 400 mg twice daily for 8 to 30 days (cycles).
  • The “Fatty Meal” Rule: For systemic infections, must be taken with a fatty meal (e.g., cheese, milk, oil). This increases absorption by 500%.
  • For intestinal worms, can be taken on an empty stomach.

Safety Warnings:

• Pregnancy (Category C): Teratogenic in animals. Contraindicated during the first trimester. WHO allows use in 2nd/3rd trimester only if benefits outweigh risks. Women of childbearing age should take it within 7 days of the start of menstruation.
• Liver Enzymes: Long-term use (for cysts) can elevate liver enzymes. Monitoring is required.
• Bone Marrow Suppression: Rare reversible leukopenia can occur with prolonged high-dose therapy.
