Dosage & Administration

• Maintenance Dose: 0.01 to 0.015 mg/kg IV administered approximately every 25 to 40 minutes to maintain surgical paralysis.
• ICU Continuous Infusion: 0.8 to 1.2 mcg/kg/min after an initial loading dose.
• Reversal: Paralysis can be rapidly reversed at the end of surgery by administering an acetylcholinesterase inhibitor (like Neostigmine, combined with Atropine or Glycopyrrolate to prevent bradycardia) or the specific encapsulating agent Sugammadex.

Safety Warnings (CRITICAL):

• BLACK BOX WARNING (Ventilatory Support): CAUSES FATAL RESPIRATORY PARALYSIS. This drug paralyzes the diaphragm. It must NEVER be administered unless facilities for intubation, artificial respiration, and oxygen therapy are immediately available.
• NO AMNESTIC OR ANALGESIC PROPERTIES: Vecuronium has zero effect on consciousness or pain. The patient will be fully awake, able to feel pain, but completely unable to move or breathe if adequate general anesthesia (like Propofol or Sevoflurane) is not simultaneously administered.
• Hepatic & Biliary Impairment: Primarily excreted in the bile. Patients with severe liver disease or biliary obstruction will experience significantly prolonged paralysis and delayed recovery.
