  • Standard Adult Dose: Typically 1.4 mg/m² (body surface area) administered once a week. Maximum single dose generally should not exceed 2 mg to minimize severe neurotoxicity.
  • Standard Pediatric Dose: 1.5 to 2 mg/m² administered once a week.
  • Administration Technique: Administered as a slow IV push over 1 minute directly into a freely running intravenous infusion (Normal Saline or 5% Dextrose) via a central line or a newly established, secure peripheral IV.

Safety Warnings (CRITICAL):

  • BLACK BOX WARNING (Fatal Intrathecal Error): FATAL IF GIVEN INTRATHECALLY. FOR INTRAVENOUS USE ONLY. Administration into the spinal fluid causes ascending paralysis, massive central nervous system destruction, and inevitable death. Syringes containing this product must be explicitly labeled with this warning.
  • Vesicant Hazard (Tissue Necrosis): Vincristine is a powerful vesicant. Extravasation (leakage into surrounding tissue) during IV administration will cause severe pain, tissue sloughing, and necrosis. Stop injection immediately if burning occurs; apply local heat and inject hyaluronidase locally per institutional protocol.
  • Neurotoxicity: Dose-limiting side effect. Causes peripheral neuropathy (loss of deep tendon reflexes, numbness, tingling), autonomic neuropathy (severe constipation, paralytic ileus), and cranial nerve palsies.
