Dosage & Administration

Recommended Dosage (Strictly as per ER Physician/Intensivist):

  • Route: Slow Intravenous (IV) Injection or Continuous IV Infusion.
  • Cardiac Arrest (Adults): Initial dose of 1 mEq/kg (usually one 50 ml vial of the 8.4% solution) administered via rapid IV push. Subsequent doses should be based on arterial blood gas (ABG) analysis.
  • Less Urgent Acidosis: Typically added to IV fluids (like 5% Dextrose) and infused slowly over 4 to 8 hours to avoid rapid shifts in cerebrospinal fluid pH.
  • Neonatal Warning: Do not use 8.4% in neonates. Must use a diluted 4.2% (0.5 mEq/ml) solution injected extremely slowly (max 1 mEq/kg/min) to prevent fatal intracranial hemorrhage from sudden hyperosmolarity.

Safety Warnings (CRITICAL):

  • Calcium Incompatibility (PRECIPITATION RISK): NEVER mix Sodium Bicarbonate with calcium-containing IV fluids (like Ringer’s Lactate or Calcium Gluconate) in the same IV line. It causes immediate, lethal precipitation of chalk-like calcium carbonate crystals that will cause fatal embolisms. Flush IV lines thoroughly.
  • Extravasation (Tissue Necrosis): Because 8.4% is intensely hypertonic, leakage outside the vein will cause severe chemical burns, tissue sloughing, and necrosis. Ensure secure IV access.
  • Sodium Overload: Contains massive amounts of sodium. Can trigger acute pulmonary edema in patients with congestive heart failure or severe renal impairment.
