Dosage & Administration

Recommended Dosage (Strictly as per WHO Malaria Guidelines):

  • Route: Slow Intravenous (IV) Infusion ONLY. (IM injection is possible but can cause severe local necrosis and abscess formation; IV is highly preferred).
  • FATAL WARNING: NEVER ADMINISTER AS A RAPID IV BOLUS. Rapid injection causes lethal hypotension and cardiac arrhythmias.
  • Standard Loading Dose: 20 mg/kg of Quinine Dihydrochloride diluted in 500 ml of 5% Dextrose, infused slowly over 4 hours.
  • Maintenance Dose: 10 mg/kg infused over 4 hours, every 8 hours, until the patient can tolerate oral antimalarial therapy.

Safety Warnings (CRITICAL):

  • Profound Hypoglycemia: Quinine strongly stimulates the pancreas to release insulin. Severe, life-threatening hypoglycemia is common, particularly in pregnant women and children. Must be infused with Dextrose and blood glucose must be monitored constantly.
  • Cinchonism: A classic toxicity syndrome presenting with tinnitus (ringing in ears), reversible hearing loss, headache, blurred vision, and nausea.
  • Cardiotoxicity / QT Prolongation: Can cause conduction disturbances. Monitor ECG if possible, and avoid co-administration with other QT-prolonging drugs (like Halofantrine or Mefloquine).
  • Blackwater Fever: A rare but severe complication causing massive intravascular hemolysis and dark urine.
