Product Overview

This formulation contains Meropenem Trihydrate, a synthetic carbapenem antibiotic with significant stability against dehydropeptidase-I (DHP-I).

The “Ultra-Broad Spectrum” Specialist:

• Mechanism (Cell Wall Destruction): Meropenem penetrates bacterial cells and binds with high affinity to Penicillin-Binding Proteins (PBPs). This inhibits the final transpeptidation step of peptidoglycan synthesis, causing the bacterial cell wall to weaken and rupture (Cell Lysis).
• Enzyme Stability: Unlike Imipenem, Meropenem is stable against human renal DHP-I and does not require a co-inhibitor (Cilastatin). It is also highly stable against most Beta-lactamases and Cephalosporinases produced by resistant bacteria.
• CNS Penetration: It has excellent penetration into the cerebrospinal fluid (CSF), making it the drug of choice for Bacterial Meningitis caused by Gram-negative bacilli.

Therapeutic Indications (Human Use)

Dosage & Administration

Recommended Dosage (Strictly as per Critical Care Specialist):

• Route: Intravenous (IV) Bolus or Infusion.
• Standard Infection: 500 mg to 1 g every 8 hours.
• Meningitis / Febrile Neutropenia: 2 g every 8 hours.
• Extended Infusion: For MDR organisms (like Pseudomonas), infusing the dose over 3 hours improves efficacy (Time > MIC).
• Renal Adjustment: Dose reduction is mandatory if Creatinine Clearance (CrCl) is < 50 ml/min.

Safety Warnings (CRITICAL):

• Seizure Risk: Although lower than Imipenem, seizures can occur, especially in patients with CNS disorders or renal failure.
• Valproic Acid Interaction: Meropenem rapidly reduces Valproic Acid levels by 60-100%, leading to breakthrough seizures. Contraindicated in patients on Valproate.
• Allergy: Contraindicated in patients with anaphylactic history to Beta-lactams.
• C. difficile: Risk of antibiotic-associated pseudomembranous colitis.
