- Mechanism (The “Controlled Release”): Unlike simple iron salts, Iron Sucrose is a complex. Upon injection, it is taken up by the Reticuloendothelial System (RES) in the liver and spleen. The sucrose shell is broken down, and the iron is slowly released to bind with Transferrin, which transports it to the bone marrow for Red Blood Cell production. This controlled release prevents “Free Iron Toxicity.”
- Dialysis Standard: It is the primary choice for CKD patients on Hemodialysis who require Erythropoietin (EPO), as EPO cannot work without adequate iron stores.
- Pregnancy Safety: It is the preferred IV iron for severe anemia in the 2nd and 3rd trimesters when oral iron is ineffective or not tolerated.

Therapeutic Indications (Human Use)

Dosage & Administration
Recommended Dosage (Strictly as per Hematologist):
- Route: Slow Intravenous (IV) Injection or Infusion ONLY.
- Standard Dose: 100 mg to 200 mg (1-2 ampoules) per session.
- Frequency: 1 to 3 times a week until total iron deficit is corrected (Calculate using Ganzoni Formula).
- Administration:
- Slow Injection: 100 mg over 2-5 minutes (undiluted).
- Infusion: Dilute 100 mg in max 100 ml Saline. Infuse over 15 minutes. (Do not dilute in large volumes or Dextrose).
Safety Warnings (CRITICAL):
- NEVER INTRAMUSCULAR (IM): ABSOLUTE CONTRAINDICATION. IM injection causes permanent brown staining (tattooing) of the skin, severe pain, and potential sarcoma risk. IV Use Only.
- Hypotension: Rapid injection causes a sudden drop in blood pressure. Inject slowly.
- Iron Overload: Monitor Serum Ferritin. Do not use if Ferritin > 500-800 ng/ml (Risk of hemosiderosis).
- Extravasation: Leakage outside the vein causes long-lasting skin discoloration. Ensure the needle is in the vein.
