• Initiation Dose: Because diarrhea is very common in the first two weeks, treatment MUST be initiated at 50 mg once daily (taken with the evening meal) for the first 2 to 4 weeks.
• Maintenance Dose: Once the patient has adjusted, the dose is increased to 50 mg twice daily.
• Duration: Treatment should be continued for at least 6 months to achieve maximum structural joint benefits.

Safety Warnings (CRITICAL):

• Severe Diarrhea & Dehydration: The most frequent adverse reaction is severe diarrhea (due to the osmotic effect of unabsorbed rhein in the colon), which can lead to dangerous dehydration and hypokalemia. If severe diarrhea occurs, Diacerein must be suspended immediately.
• Hepatotoxicity: Elevated liver enzymes and severe hepatic injury have been reported. Regular monitoring of liver function tests (LFTs) is mandatory, and the drug is strictly contraindicated in patients with current or prior liver disease.
• Urine Discoloration: Patients must be counseled that Diacerein causes an intense yellow or brownish-red discoloration of the urine. This is a harmless metabolic effect but can cause extreme patient anxiety if not pre-warned.
