Dosage & Administration

Recommended Dosage (Strictly as per Physician):

  • Duodenal Ulcer (Acute): 40 mg once daily at bedtime OR 20 mg twice daily. Treatment duration: 4–8 weeks.
  • Duodenal Ulcer (Maintenance): 20 mg once daily at bedtime.
  • Benign Gastric Ulcer: 40 mg once daily at bedtime.
  • GERD: 20 mg twice daily for up to 6 weeks.
  • Renal Impairment (Critical): In patients with creatinine clearance < 50 mL/min, the dose must be reduced to 20 mg daily or the interval prolonged to 36–48 hours to avoid CNS toxicity.

Safety Warnings:

  • Renal Dose Adjustment: Famotidine is excreted largely unchanged by the kidneys. Accumulation in renal failure can cause confusion, delirium, and seizures.
  • CNS Effects: Elderly patients are more susceptible to confusion and dizziness.
  • Malignancy Masking: Symptomatic response to therapy does not preclude the presence of gastric malignancy. Warning signs (weight loss, dysphagia) should be investigated.
