Dosage & Administration

Recommended Dosage (Strictly as per Cardiologist – High Toxicity Risk):

  • Route: Slow Intravenous (IV) Injection ONLY. (IM is painful and absorption is erratic – Avoid).
  • Digitalizing (Loading) Dose: 0.25 to 0.5 mg (250-500 mcg) IV over 5-10 minutes. May repeat 0.25 mg every 4-6 hours to a total of 1.0 mg.
  • Maintenance Dose: 0.125 mg to 0.25 mg daily (Oral or IV).
  • Renal Impairment: Drastic dose reduction required. Digoxin is excreted by kidneys. Reduce dose by 50-75% in renal failure to prevent fatal toxicity.

Safety Warnings (CRITICAL):

  • Digoxin Toxicity: Symptoms include nausea, vomiting, visual disturbances (yellow/green halos around lights), and life-threatening arrhythmias (heart block, VT).
  • Hypokalemia (Low Potassium): Low potassium increases Digoxin toxicity. Patients on Diuretics (Furosemide) are at high risk. Serum Potassium must be normalized before administration.
  • Monitor Heart Rate: Withhold dose if Pulse < 60 bpm.
