Product Overview

This formulation combines Cefpirome, a fourth-generation zwitterionic cephalosporin, with Sulbactam, a beta-lactamase inhibitor with unique intrinsic activity.

• Mechanism (Rapid Penetration): Cefpirome’s zwitterionic structure allows it to pass through the outer membrane of Gram-negative bacteria faster than older cephalosporins. Once inside, it binds to Penicillin-Binding Proteins (PBPs) to stop cell wall synthesis.
• The Sulbactam Advantage (Acinetobacter): While Cefpirome is naturally stable against many enzymes, adding Sulbactam extends the spectrum to include anaerobes (*Bacteroides*) and provides direct activity against Acinetobacter baumannii, a critical pathogen in ventilator-associated pneumonia (VAP) that Cefpirome alone might miss.
• Neutropenia Defense: This combination is widely used as empirical therapy for Febrile Neutropenia (fever in cancer patients with low immunity) because it covers both resistant Staph (Gram-positive) and Pseudomonas (Gram-negative).

Dosage & Administration

Recommended Dosage (Strictly as per Physician):

• Route: Intravenous (IV) Infusion Preferred. (Can be given Deep IM).
• Adults: 1.5 g (1g + 0.5g) every 12 hours. In severe infections, every 8 hours.
• Neutropenia/Pseudomonas: Dosing frequency may be increased to every 8 hours.
• Renal Impairment: Dose adjustment is mandatory if Creatinine Clearance < 50 ml/min.
• Reconstitution: Dissolve 1.5g vial in 10-20ml WFI/Saline. Shake well. Carbon dioxide release is normal (vent the vial).

Safety Warnings (CRITICAL):

• Penicillin Allergy: CONTRAINDICATED in patients with a history of anaphylaxis to penicillins or cephalosporins.
• Nephrotoxicity: Monitor renal function closely, especially when used with aminoglycosides or loop diuretics.
• Neurotoxicity: High doses in renal failure patients can cause encephalopathy or seizures.
