Dosage & Administration
Recommended Dosage (Strictly as per Cardiologist/Surgeon):
- Route: Deep Subcutaneous (SC) Injection. (Abdominal wall). NEVER IM.
- Prophylaxis (High Risk Surgery): 40 mg SC once daily.
- Treatment (DVT/PE/ACS): 1 mg/kg body weight every 12 hours (e.g., 80mg for an 80kg patient).
- Renal Impairment: Dose adjustment required if Creatinine Clearance < 30 ml/min (Use 1 mg/kg once daily).
- Technique: Do not expel the air bubble from the syringe before injection (prevents drug loss).
Safety Warnings (CRITICAL):
- Epidural/Spinal Hematoma: BLACK BOX WARNING. Patients receiving neuraxial anesthesia (spinal/epidural) while on LMWH are at risk of spinal hematoma, which can result in long-term or permanent paralysis.
- Bleeding Risk: Use with extreme caution in patients with active hemorrhage or low platelet counts.
- HIT (Heparin-Induced Thrombocytopenia): Monitor platelet counts. Discontinue if platelets drop significantly.
