- Hepatic Impairment: Dose reduction required in severe liver disease (Max 2g/day).
- Renal Impairment: No adjustment needed unless renal failure is severe (< 18 ml/min).
Safety Warnings (CRITICAL):
- Disulfiram-like Reaction (ALCOHOL WARNING): The NMTT side chain blocks alcohol metabolism. Drinking alcohol within 72 hours of injection causes severe vomiting, flushing, and tachycardia. Patients must be warned strictly.
- Bleeding Risk (Hypoprothrombinemia): The NMTT chain can inhibit Vitamin K production in the gut, leading to bleeding. Vitamin K supplementation is recommended for patients at risk (elderly, malnourished).
