Dosage & Administration
Recommended Dosage (Strictly as per Physician – Monitor aPTT):
- Route: Intravenous (IV) Bolus/Infusion or Deep Subcutaneous (SC) Injection.
- NEVER INTRAMUSCULAR (IM): Due to high risk of massive hematoma formation.
- Full Anticoagulation: 5,000 units IV Bolus, followed by 20,000–40,000 units/24h via continuous infusion.
- Monitoring: Dose must be adjusted to maintain aPTT at 1.5 to 2.5 times the control value.
Safety Warnings (CRITICAL):
- HIT (Heparin-Induced Thrombocytopenia): A serious immune reaction that drops platelet counts. Monitor platelet counts every 2-3 days.
- Bleeding: Hemorrhage can occur at any site. Monitor for signs of occult blood.
- Osteoporosis: Long-term use (>3 months) can lead to bone density loss.
- Benzyl Alcohol: Vials containing Benzyl Alcohol should not be used in neonates (“Gasping Syndrome”).
