Dosage & Administration

Recommended Dosage (Strictly as per Endocrinologist):

  • Route: Oral.
  • The “First Bite” Rule (CRITICAL): Acarbose must be chewed with the first bite of the meal or swallowed whole with a little liquid immediately before the meal. If taken after the meal, it is ineffective because the enzymes have already started working.
  • Titration (To reduce Flatulence):
    • Start with 25 mg once daily (dinner) for 1-2 weeks.
    • Increase to 25 mg three times daily.
    • Titrate up to 50 mg or 100 mg three times daily as tolerated.

Safety Warnings:

  • GI Side Effects (Very Common): Because undigested carbohydrates reach the colon, bacteria ferment them, causing flatulence (gas), bloating, and diarrhea. This usually subsides after 4-8 weeks as the gut adapts.
  • Hypoglycemia Treatment: If a patient gets low blood sugar while on Acarbose, they must use Glucose (Dextrose) tablets or gel. Sucrose (Table Sugar) or Juice will NOT work fast enough because Acarbose blocks the breakdown of sucrose.
  • Liver Enzymes: Rare elevations in liver enzymes can occur at high doses (200mg+). Monitoring is recommended.
  • Contraindications: Inflammatory Bowel Disease (IBD), colonic ulceration, or partial intestinal obstruction.
