• Synergy (The “Double Hit”): It is almost always used in combination with a sulfonamide (like Sulfadiazine or Sulfadoxine). While the sulfonamide blocks the synthesis of folate, Pyrimethamine blocks the activation of folate. This sequential blockade provides a synergistic effect that is far more lethal than either drug alone.

Dosage & Administration

Recommended Dosage (Strictly as per Infectious Disease Specialist):

• Route: Oral.
• Toxoplasmosis (Adults):
  • Loading: 50 mg to 75 mg daily (with Sulfadiazine) for 1–3 weeks.
  • Maintenance: Reduced to 25 mg daily for several weeks.
• Malaria Prophylaxis: 25 mg once weekly.
• Leucovorin Rescue (CRITICAL): High doses of Pyrimethamine inhibit human folate too. It is mandatory to co-administer Folinic Acid (Leucovorin) during high-dose therapy to prevent bone marrow failure (anemia/low platelets).

Safety Warnings:

• Bone Marrow Suppression: Can cause megaloblastic anemia, leukopenia, and thrombocytopenia due to folate deficiency. Regular blood counts (CBC) are required.
• Rash (SJS): When combined with sulfonamides, there is a risk of severe skin reactions like Stevens-Johnson Syndrome. Discontinue at the first sign of rash.
• Pregnancy: Teratogenic in the first trimester (folate antagonist). Use only if benefits outweigh risks, and always with folate supplementation.
