  • Anesthesia Reversal: In surgery, muscle relaxants (like Rocuronium or Vecuronium) block the NMJ to paralyze muscles. Neostigmine floods the junction with ACh, displacing the relaxant and restoring muscle strength so the patient can breathe on their own again.
  • Myasthenia Gravis: It improves muscle tone in patients with this autoimmune disease by ensuring maximum utilization of the few available acetylcholine receptors.

Dosage & Administration

Recommended Dosage (Strictly as per Anesthesiologist):

  • Route: Intravenous (IV), Intramuscular (IM), or Subcutaneous (SC).
  • Reversal of Anesthesia (IV): 0.03 mg/kg to 0.07 mg/kg.
    • CRITICAL CO-ADMINISTRATION: Neostigmine causes severe bradycardia (slow heart rate). It MUST be administered with an anticholinergic like Atropine (0.6–1.2 mg) or Glycopyrrolate (0.2–0.6 mg) to prevent cardiac arrest.
  • Myasthenia Gravis: 0.5 mg to 2.5 mg IM/SC every 1-3 hours depending on response.

Safety Warnings (CRITICAL):

  • Bradycardia/Cardiac Arrest: As noted above, never give IV Neostigmine without Atropine/Glycopyrrolate unless the patient is already tachycardic.
  • Bronchoconstriction: Increases bronchial secretions and constriction. Use with caution in asthmatics.
  • Mechanical Obstruction: CONTRAINDICATED in patients with mechanical intestinal or urinary obstruction (can cause rupture).
