  • Re-treatment: 25 mg/kg body weight daily for 60 days, then reduce to 15 mg/kg.
• Renal Impairment: Ethambutol accumulates in kidney failure. Dose adjustment is mandatory based on Creatinine Clearance (usually given every 24-36 hours).
• Administration: Can be taken with food if gastric irritation occurs, although absorption is slightly better on an empty stomach.

Safety Warnings:

• Optic Neuritis (CRITICAL): The most serious side effect is retrobulbar neuritis, causing decreased visual acuity and loss of Red-Green color discrimination.
  • Protocol: Patients must have a baseline eye exam before starting and report any blurred vision immediately. Not recommended for children too young to report visual changes.
• Hyperuricemia (Gout): Ethambutol reduces the renal excretion of uric acid, which can trigger acute gout attacks.
